The GLP-1 Era — and Its Hidden Cost
12.4% of U.S. adults are now taking a GLP-1 medication for weight loss — more than double the rate of just two years ago. Tirzepatide (Zepbound, Mounjaro) overtook semaglutide (Wegovy, Ozempic) as the most-prescribed GLP-1 in late 2025. The numbers on the scale are real: 15–20% body weight lost in landmark trials.
But there is a catch the early marketing did not advertise. Up to 25–40% of the weight you lose on a GLP-1 is muscle, not fat — and women, especially those over 45, lose proportionally more lean mass than men. That is the population we treat every day at RevitalyzeMD.
Why "Ozempic Face" Is Really an Ozempic Body Problem
The drugs work by suppressing appetite. That is exactly the issue. When you eat dramatically less, your body breaks down whatever is most expensive to maintain — and muscle is metabolically expensive. Without a deliberate plan to protect it, the body cannibalizes lean tissue along with fat.
The result shows up as:
- Clinical Note: A gaunt, hollowed face — the now-famous "Ozempic face."
- Clinical Note: A lower resting metabolism, which sets up regain the moment the medication stops.
- Clinical Note: Loss of strength, balance, and bone density — all serious issues for perimenopausal and postmenopausal women.
And then there is the regain problem. The STEP 1 extension trial followed semaglutide patients for a year after they stopped the drug. They regained roughly two-thirds of the weight they had lost. Net loss fell from 17.3% at week 68 to just 5.6% at week 120.
The Peptide Stack That Protects What You Build
This is where we change the equation. Instead of using a GLP-1 alone — and watching muscle disappear with the fat — we layer in growth-hormone-axis peptides that signal your body to preserve and rebuild lean tissue while you are in a caloric deficit. The science is older than the GLP-1 craze; we're simply using it for a new problem.
What the Research Actually Shows
- Clinical Note: Wilding et al., Diabetes, Obesity & Metabolism (2022) — the STEP 1 extension that documented two-thirds weight regain within a year of stopping semaglutide. The case for an exit strategy, not just a starting strategy.
- Clinical Note: Prado et al. (2024) — review of lean body mass changes with GLP-1 therapies. Identifies resistance training, adequate protein, and growth-hormone-axis support as the three levers that protect muscle on these drugs.
- Clinical Note: Stanley et al., JAMA (2014) — tesamorelin reduced visceral adipose tissue and liver fat in central obesity without worsening glucose. Foundation data for GH-axis peptides in body recomposition.
The Affordability Cliff Coming in 2026
The economics are also shifting. Commercial coverage for Wegovy and Zepbound dropped sharply in 2025. Medi-Cal and Pennsylvania Medicaid eliminated weight-loss GLP-1 coverage on January 1, 2026. CVS Caremark dropped Zepbound from its formulary. Cash retail runs $900–$1,400 per month; direct manufacturer programs land at $299–$349; telehealth compounded versions, $149–$399.
Many of our patients ask the same question: "How do I get the result without staying on a $14,000-a-year medication forever?" The answer is rarely the GLP-1 alone. It is the protocol around it.
How We Build a Stack at RevitalyzeMD
We are not anti-GLP-1. They are remarkable tools when used correctly. We are anti–blunt-instrument GLP-1, where the drug does the work and the patient quietly loses the body composition they came in with.
Our approach:
- Clinical Note: Comprehensive labs first — hormones (estradiol, progesterone, testosterone, thyroid), metabolic panel, IGF-1, inflammatory markers, body composition scan.
- Clinical Note: The right tool, at the right dose — sometimes that's a low-dose GLP-1 paired with peptides; sometimes it's peptides alone; sometimes it's hormone optimization first because the metabolism was broken upstream.
- Clinical Note: Strength training and protein non-negotiable — 1 gram of protein per pound of goal body weight, three to four resistance sessions weekly.
- Clinical Note: Re-test every 8–12 weeks — adjust dosing, swap peptides, taper the GLP-1 once goals and body composition are locked in.
The goal is not to be on a medication forever. The goal is to get to a leaner, stronger, more resilient version of yourself — and to keep her there long after the prescription ends.
